Registered Nurse

A healthcare organization in Grand Rapids, MI, is seeking a dedicated Home Health Registered Nurse (RN) to join their team. This role involves providing high-quality, compassionate care to patients in the comfort of their homes.

About the Opportunity

• Hybrid or Remote:** Onsite 1x a week at the Grand Rapids office; field visits required.
• Shift: Monday to Friday
• Schedule: 8 AM - 5 PM
• Hours: Full-time
• Setting: Home visits and assisted living facilities

Responsibilities:

• The Home Health Registered Nurse (RN) will be responsible for:
• Managing a caseload of home health care patients and their families.
• Providing comprehensive assessments and care plans.
• Ensuring patient and staff confidentiality as per HIPAA regulations.
• Maintaining accurate and complete patient documentation.
• Coordinating with interdisciplinary teams to deliver quality care.
• Evaluating patient progress and modifying care plans as needed.
• Communicating significant health changes to the appropriate personnel.

Qualifications:

• Minimum 1 year of experience in home health, hospice, assisted living, or long-term care.
• Valid RN license; BSN not required.
• Proficient in basic computer skills and EMR systems.
• Strong communication skills and the ability to work independently.
• Current valid driver's license and reliable transportation.
• Ability to travel within the service area as needed.

Desired Skills:

• Experience with HomeCare HomeBase or OASIS is advantageous.
• Familiarity with adult patient populations, particularly 50+.
• Ability to handle confidential information professionally.
• Previous experience in a similar healthcare setting preferred.
